Instructions

Students are required to identify ONE (1) of the facilities available within your neighbourhood that supports care transition.

The report must include the following:

Assignment information

• This individual assignment carries a weightage of 35%.
• The due date of this individual assignment is in Week 7, Monday, 1 Dec 2025, 2359 hrs via Brightspace (BS).
• The word limit of this individual assignment is 800 +/- 10%

Submission of Assignment

i) Main Assignment

• • Save your assignment separately from the end references list as a MS Word file
  • Label file as “Assignment _your Name (as in Register)_Adm No.”
  • Always check for originality report of your work

ii) End reference list

• • End reference list format must be in accordance with APA reference
  • Include a complete end reference list after your main assignment. No need to have a separate file for your end reference.

Information on assignment submission

1) Students are required to submit the individual assignment in Word document (Do not pdf your file)

2) Acceptable font types are Calibri or Arial with font size Use 1.5 line spacing.

3) Assignments must be presented in accordance with Nanyang Polytechnic SHSS Guide to Presentation of Assignment, which is available in 101 SHSS Student Resources in BS.

4) Plagiarism (i.e. copying another person’s work without proper referencing) and abetment to plagiarism (i.e. sharing of your work with others) are strictly prohibited and subject to disciplinary action, if/when committed.

5) Use of AI tools plagiarism

“Taking and using the whole or any part of ideas, words or works of others, including contents generated by AI tools and passing it off as one’s own work without acknowledgement of the original source. “

The ‘acknowledgement of the original source’ must not exceed 50% of the original source. If the assignment submitted is substantially generated by AI, investigation will be conducted as this constitute a breach of Academic Integrity Policy.

6) Similarity Index

Students are required to submit their assignment to Turnitin in BS and to ensure that the similarity index (SI) is kept ≤20%.

a. Take note that the latest submission will override the previous

b. SI report will be generated after 24 hours for the fourth and subsequent

c. No alternative attempt will be given if

i. students do not resubmit assignment despite SI >20%, or

ii. no SI report is generated in Turnitin (If the SI report takes more than 1 hour to generate, it is the student’s responsibility to inform respective module tutor for assistance before the due date).

d. Students are required to keep a copy of the assignment submitted and the acknowledgement email of assignment submission as proof of work upon request.

7) Late Submission Penalty Guidelines

Submission of an assignment will be considered late if the submission is received after the stipulated due date and time. If the submission is received within 5 calendar days of the due date, the following penalty applies:

• If the learner passes, the score will be capped at 50% of the base
• If the learner fails, the prevailing scores will be computed as the final

An assignment submitted after 5 calendar days of the due date will be reviewed for feedback but will be awarded a zero mark.
