Jane Jenkins, the Safety Officer has become alarmed at the increase in the number of accidents. Not only is she concerned about the health and safety of employees, but she is worried about the loss of productivity and the increase in insurance premiums that will inevitably follow if this trend continues.
Jane sees no apparent connection between the accidents since they involve different people from various departments where the accidents have taken place.
